For the health of our territory
By joining together in CPTS (Territorial Professional Health Community), health professionals and medico-social actors in your territory organize themselves around a collective health project in order to meet the needs of the population andimprove our health services.
Our key actions
The CPTS Val de Guyennehealth project, consistent with the needs of the territory, is structured around 4 core missions:
Access to healthcare
- Facilitating access to a primary care physician
- Facilitating access to a gynecologist
- Improving the management of unscheduled care
Multi-professional care pathways centered around the patient
- Coordination of healthcare professionals and link between town and hospital
- Creation of a territorial directory
- Coordinated management of elderly or frail individuals
- Implementation of palliative care pathways
Prevention
- Raising awareness about cancer screenings
- Improve vaccination coverage
- Supporting the deployment of patient therapeutic education programs (diabetes and cardiovascular)
- Information and awareness-raising on nutrition and malnutrition
Health Crisis Management
- Drafting a plan to respond to exceptional health crises
Medical & Gynecological Bus: a mobile medical clinic
The Gynecologist & Medical Bus is an innovative initiative to facilitate access to a gynecologist or midwife in the municipalities of the Lauzun community of municipalities.
Equipped for consultations, it allows users to be received for:
- prevention and screening consultations,
- gynecological consultations.
- It will soon allow for general medical consultations.
